About Us

The Listowel-Wingham and Area Family Health Team (LWA FHT) is a not-for-profit organization, funded by the Ministry of Health to provide primary care services to the residents of North Perth and North Huron and area through various healthcare services and programs. The LWA FHT amalgamated on April 1, 2021. Prior to this amalgamation, the two predecessor Family Health Teams (North Perth FHT and North Huron FHT) worked very closely together since their inceptions in approximately 2008.

We employ approximately 30 people, including Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Social Workers and Mental Health Counsellors, Pharmacists and Dietitians.

The FHT partners with other local health agencies in carrying out many of our programs. These partnerships are highly valued, as they not only improve and enhance the quality of the programs and services. We are members of the Huron-Perth and Area Ontario Health Team (HPA OHT), a collaborative of 60+ healthcare organizations in Huron and Perth whose vision is to create “a sustainable people-driven system that strives to provide a positive experience for all.”

Land Acknowledgement

We acknowledge and give thanks for the land on which we gather as being the traditional territory of the Haudenosaunee (hoe-dee-no-SHOW-nee)/People of the Long House and the Anishinaabe (An-ish-in-aabe).

We recognize the First Peoples’ continued stewardship of the land and water, and that this territory is subject to the Dish with One Spoon Wampum under which multiple nations agreed to care for the land and resources by the Great Lakes in peace.

We also acknowledge and recognize the treaties signed in regard to this land including Treaty #29 and Treaty #45 ½. Our roles and shared responsibilities as treaty people mean we are committed to moving forward in reconciliation with gratitude and respect with all First Nations, Métis, Inuit and Indigenous Peoples.
