Description

The Congestive Heart Failure (CHF) program is multi-regional program, developed to provide advanced, specialized care to the population of Huron and Perth. This program is headed by Stratford Internal Medicine Associates. This group of internal medicine physicians travel to regional hospitals and work with local nurse practitioners to provide care to patients with heart failure. The goal is to provide access to specialized care to patients who may have limited access to other cardiac services.  The nurse practitioners and internists work with the patients and their family physician in a collaborative way to provide not only assessment, diagnosis and treatment; but also ongoing monitoring for those with CHF.

How do I join?

If you are a rostered patient and in need of mental health/counselling supports please speak to your health care provider about a referral, or you may request this service on your own by calling in to the clinic.
