Anyone with a newly diagnosed or long-standing type 1, type 2, prediabetes or gestational diabetes who is over the age of 18.

Description

Our teams provide assessment & information in individual appointments where you will meet with either a Registered Dietitian, Registered Nurse, Registered Pharmacist or a combination depending on the complexity of your diabetes & your diabetes care needs. Some topics we may cover include:

  • Types of diabetes and physiology
  • Self-monitoring of blood glucose
  • Physical activity
  • Nutrition
  • Medication and insulin
  • Proper foot care
  • Hypoglycemia treatment
  • Sick day management
  • Prevention/management of complications related to diabetes

 

Our location is also connected with the Huron Perth Diabetes Program

How do I join?

Anyone interested can contact the Family Health Team reception desk to book an appointment, or can be referred to the program by their family doctor or nurse practitioner.  However, referrals from a family doctor or nurse practitioner are not necessary.
