Description
Our teams provide assessment & information in individual appointments where you will meet with either a Registered Dietitian, Registered Nurse, Registered Pharmacist or a combination depending on the complexity of your diabetes & your diabetes care needs. Some topics we may cover include:
- Types of diabetes and physiology
- Self-monitoring of blood glucose
- Physical activity
- Nutrition
- Medication and insulin
- Proper foot care
- Hypoglycemia treatment
- Sick day management
Prevention/management of complications related to diabetes
